In order to complete your computer repair in the most thorough and efficient
manner, certain information is required. Please fill out the customer information
form completely, sign it, and include it with your equipment. Failure to provide
complete information can result in repair delays and/or additional expense.     

Please do not drop off a computer at any location without calling me first.

On-site Service Information Form
Use this form for on-site service to describe your problem and provide
necessary information. Having this form filled out ahead of time will save
time and result in more economical service.
